Fellow - $250.00 (USD) Subscription period: 1 year, on: January 1st Automatic renewal (recurring payments) Physicians licensed in the State of Indiana, whose practice or professional activities are limited to dermatology, dermatopathology, and/or dermatologic surgery. Must be board-certified in dermatology by the American Board of Dermatology, the American Osteopathic Board of Dermatology, or a non-U.S. Board.

Associate - $250.00 (USD) Subscription period: 1 year, on: January 1st Automatic renewal (recurring payments) Completed an ACGME accredited dermatology residency program and are qualified to take the examination of the American Board of Dermatology.

Affiliate - $250.00 (USD) Subscription period: 1 year, on: January 1st No automatically recurring payments Board-certified pathologist with subspecialty board certification in dermatopathology.

Adjunct - $50.00 (USD) Subscription period: 1 year, on: January 1st Automatic renewal (recurring payments) American Board of Dermatology or American Osteopathic Board of Dermatology certified physicians who do not reside in Indiana.
